- Connect Network (State hospital-linked network)
- In hospital
- Hospitalisation at State hospitals only via the Connect Network arrangement; no overall annual limit; 30% co-payment on hospital account if network not used (Connect/Ingwe Network variants only); specialists covered at 100% of Momentum Medical Scheme Rate. Contribution is income-banded (R645–R4,546/month depending on income) — premium shown is the typical working-income band.

- None (optional HealthSaver+ addon available)
- Chronic medication
- 26 CDL conditions, no annual limit. State formulary via State facilities.
- Maternity
- No overall annual limit on confinement/neonatal ICU (emergency C-sections only). Day-to-day maternity programme available (health assessments, baby immunisations, maternity programme via any provider).
- Dental
- 1 basic dental consultation/year (exams, fillings, x-rays per formulary); R125 co-payment. No specialised dentistry.
- Optical
- 1 eye test + 1 pair of standard lenses every 2 years per beneficiary (refraction > 0.5 required).
- Mental health
- Limited to Prescribed Minimum Benefits (PMB) at State facilities, in- and out-of-hospital.
